Provider Agreement, Rate and Banking Information Forms. These forms must be submitted using this website
in order to receive payment for child care services provided to eligible families. |
This message is to notify all users that the CCIDS Portal will be shut down permanently on 10/31/2024. After 10/31/2024 all users of the CCIDS provider portal will no longer have access to the system. Any information obtained previously by way of using the CCIDS portal, is available in the KinderConnect system located at KinderConnect Logon (ohiocctap.com). Thank you.
