The purpose of this site is to allow online access for eligible child care providers to complete, submit, view and update the
Provider Agreement, Rate and Banking Information Forms. These forms must be submitted using this website
in order to receive payment for child care services provided to eligible families.
Authorizations will end in Ohio Electronic Child Care (PWeb) on December 15, 2018 at 11:59pm for current attendance. Back swipes can be completed through December 29, 2018 for services provided 12/2/2018 12/15/2018. The Conduent-Only POS equipment that is currently assigned must be returned. The signed agreement with providers requires you to either return the device(s) or pay $330.00 per device. Please call 1-888-516-4776 to request a return packet for the Conduent equipment.
